OUPV question

This is a question for those guys who have obtained their OUPV licenses.

How did you document your currency and service?

For the OUPV limited, 90 days of service is required within 3 years of application.

How would I go about documenting 90 days on the water?

Is there an official format that is required?

    Thanks!

    You need form CG-719S.

Documentation was easy for me as I was able to just got back and look through my scheduling book to determine number of days on the water and could prove those dates if needed by being able to produce bank record and expense receipts if needed.

You can document your hours through the use of a logbook if you can prove ownership (registration, title, sales receipt etc.) of a vessel during the time in question.

If you did not own a vessel during the timeframe in question you have to produce a notarized statment from a vessel owner verifying your days of service.

I would avoid going for the limited OUPV and go for the whole enchilada the only difference is the 360 days of service vs. 90 days.
