Attention Commercial Drivers License Holders

I was completely unaware that any type of communication had been sent out pertaining to a form that is to be filled out and returned to DPS by the end of January. According to the person I talked to, they will remove your CDL if you have not filled out and returned this Commercial Drivers License Self Certification Form. I went through the same wake up call a few months ago and all one needs to do is provide a photocopy of both sides of your (current) health certificate along with the form at a dmv office and they will punch in the info and your good to go without getting charged a fee.My licsense had been reduced to a “D” rating but was restored within 5 minutes after they entered the info.Lucky I did not get caught in a truck before I found out about it.

Same here in Illinois. Have until Jan 30, 2014.

You need to do it now every time you renew your license or get a new medical card. First time I had to do it was when I renewed my license back in Oct 2012 and then again this month with my new medical certificate. You can check your status here for a MN DL:
