Weird! But it doesn’t look like a mouth to me, but more like a hole. I think that guy may have been a heavy smoker at one point in his life.

That’s what I thought too, but if you look really close the second mouth has its own set of (very small, not-completely-formed) barbels.
That’s some bizarre stuff.
